Assessments provide us with information that will help us determine the supports and services necessary to help individuals in their rehabilitation and recovery. The purpose of this module is to introduce you to the different types of assessments that are done, and how they relate to the process of recovery.  
Course Objective:
• Describe the purpose of assessments.
• List the different types of assessments and the information gathered in each.
• Describe or record examples of behavior in an objective manner.
• Describe the importance of clearly defining and recording behaviors to respond to the needs, desires, and interests of each participant
• Define informal and formal assessments

Direct support professionals, case managers, nursing assistants, health aides
