Description:
Clinical Documentation Basic – Permanent Supportive Housing (PSH) Programs | 8 hours 

The course is designed for Permanent Supportive Housing (PSH) Program Staff (outreach specialists, peer recovery specialists, housing stability specialists, etc.) to address proper documentation methods for agency intake, assessments, housing service plans, reviews, progress notes, and discharge summaries according to Agency and credentialing standards and expectations, and PSH program requirements, procedures, and criteria.


Objective:
Objectives: 
-General documentation overview
-Review of expectations of documentation practices
-Person Centered Language practices
-Facilitating an agency intake with Profiler demonstration and practice
-Conducting and documenting the Intake Assessment with Profiler demonstration, practice, and feedback
-Development and documentation of the housing service plan with Profiler demonstration, practice, and feedback
-Writing of Progress Notes 
-Documentation of Discharge Summary
